This resource from Million Hearts® 2022 provides a list of process improvements for increasing cardiac rehabilitation uptake in eligible patient populations. Cardiac rehabilitation is an evidence-based prevention program for improving cardiovascular health after a cardiac-related event or procedure. This change package provides:
- Guidance on identifying changes to guide process improvement
- Evidence-based and practice-based tools
- Resources for implementing the tools
