In the past 12 months, Have you delayed getting medical care because of the cost? Was there any time when you needed medical care, but did not get it because of the cost? Have you skipped medication doses to save money? Took less medication to save money? Delayed filling a prescription to save money? Was there any time when you needed prescription medication, but did not get it because of the cost? Have you delayed getting dental care because of the cost? Was there any time when you needed dental care, but did not get it because of the cost? Have you delayed getting counseling or therapy from a mental health professional because of the cost? Was there any time when you needed counseling or therapy from a mental health professional, but did not get it because of the cost?

Item responses are "No – no difficulty, Yes – some difficulty, Yes – a lot of difficulty, or Cannot do at all."

The next questions ask about difficulties you may have doing certain activities because of a health problem. Do you have difficulty seeing, even if wearing glasses? Do you have difficulty hearing, even if using a hearing aid? Do you have difficulty walking or climbing steps? Do you have difficulty remembering or concentrating? Do you have difficulty (with self-care such as) washing all over or dressing? Using your usual (customary) language, do you have difficulty communicating, for example understanding or being understood?
