This method was used because it was a statistically significant improvement from the baseline, and many factors can affect costs associated with primary health care.

Numerator:

Item responses are "Yes, No, Refused or Don't Know."

In the past 12 months, Have you delayed getting medical care because of the cost? Was there any time when you needed medical care, but did not get it because of the cost? Have you skipped medication doses to save money? Took less medication to save money? Delayed filling a prescription to save money? Was there any time when you needed prescription medication, but did not get it because of the cost? Have you delayed getting dental care because of the cost? Was there any time when you needed dental care, but did not get it because of the cost? Have you delayed getting counseling or therapy from a mental health professional because of the cost? Was there any time when you needed counseling or therapy from a mental health professional, but did not get it because of the cost?
Denominator:

Item responses are "No – no difficulty, Yes – some difficulty, Yes – a lot of difficulty, or Cannot do at all."

The next questions ask about difficulties you may have doing certain activities because of a health problem. Do you have difficulty seeing, even if wearing glasses? Do you have difficulty hearing, even if using a hearing aid? Do you have difficulty walking or climbing steps? Do you have difficulty remembering or concentrating? Do you have difficulty (with self-care such as) washing all over or dressing? Using your usual (customary) language, do you have difficulty communicating, for example understanding or being understood?

Methodology notes

Adults with disabilities are considered to experience delays in care due to cost if they or an adult proxy respond "yes" to any of the questions listed in the numerator section. Adults with disabilities are identified in this objective if they or an adult proxy responds "yes" to questions used to identify people with disabilities.

Changes between HP2020 and HP2030
This objective differs from Healthy People 2020 objective DH-4 in that objective DH-4 tracked adults with disabilities who experienced delays in receiving care due to specific barriers, while this objective tracks adults with disabilities who experience delays in receiving care due to cost as a barrier. Additionally, DH-4 used the American Community Survey (ACS) questions, while this objective uses the Washington Group short set of disability questions.

  • Revised. 

    In 2021, due to the 2019 NHIS redesign, the baseline was revised from 37.2% in 2018 to 53.2% in 2019. The target was revised from 34.9% to 48.4% using the original target setting method.

    In 2024, the following categories were recalculated with the use of a newer variable: Never married, American Indian or Alaska Native only, Native Hawaiian or Other Pacific Islander only, 2 or more races only, Not Hispanic or Latino American Indian or Alaska Native only, Not Hispanic or Latino Native Hawaiian or Other Pacific Islander only, Not Hispanic or Latino 2 or more races only. Estimates from data year 2022 or earlier may vary slightly from estimates previously reported. The baseline, baseline year and target were not affected.
